Am I Covered By Insurance?All the services that we provide can be covered by Health Benefits Plans. Your plan is specfic to your company, so you should check with your employer or insurance provider so you understand the details of your coverage. We cannot speak to your insurance company for you. Questions to Ask: Is my coverage based on the calendar year or a 12 month cycle? How much annual coverage do I have per treatment type? What is the coverage limit per treatment? Do I need a Medical Doctor referral? How to I get the referral to my insurance company? Does my plan allow Direct Billing? Does my plan allow Assignment of Benefits to the clinic or will I have to pay the clinic?

Do You DIRECT BILL To Insurance?YES! We DIRECT BILL to most insurance companies. Not sure of the details of your insurance plan? We do not have access to this information, so we recommend that you call your insurance company before your first appointment with us. We will submit on your behalf while you are having your treatment. If for some reason, you are not covered for the full amount or the submission does not go through, you will be responsible for any unpaid portion. Your insurance company requires you to complete a Direct Billing Form before your appointment. Each patient covered by your plan must complete their own Direct Billing form. Our forms are digital and will be emailed within your confirmation email. All our forms will return to us automatically once completed. Special note for SunLife plan members. The plan member must complete the specific SunLife Direct Billing Form for each person covered under your plan. Why Do I Need To Fill In A New Patient Form?We take your health seriously and follow all the professional standards of patient care as recommended by our governing colleges. Our practitioners are required to perform a detailed health history at your first appointment. This includes reviewing the New Patient Form that you have completed, asking additional questions for clarification and performing the required physical assessment. This diligence with your health history ensures that you are given the appropriate treatment. At following treatments your practitioner will quickly review to see if there have been any significant changes to your health since the last treatment.

What Information Should I Bring?If you have had a significant health event like an accident, surgery or any severe health condition or complication, you can bring any medical reports, bloodwork, x-rays or MRIs to your first appointment. We will take the time to include this health information in your assessment. If you are a current patient and have had a signficant health change, please let us know and bring any medical paperwork about it that you may have. Even better! Get us those reports to us ahead of your appointment. You can email or use "Secure Docs" to get them to us in advance - info@bramptonwellness.ca
