Why Do I Need To Fill In A New Patient Form?
We take your health seriously. All of our practitioners are required to perform a detailed health history at your first appointment. This includes reviewing the new patient form that you have filled in, asking additional questions for clarification and performing the required physical assessment. This diligence with your health history ensures that you are given the appropriate treatment. At following treatments your practitioner will quickly review to see if there have been any significant changes to your health since the last treatment.
